Continental Western’s Fire-Pak policy has a 3-year rate guarantee. If the 3 year guarantee is expiring, Continental Western Underwriting requires some renewal applications be completed. Please note that General Liability coverage is based on the square footage of the fire hall. Even if there is no property coverage for the fire hall on the FDK policy, please complete question number 1.
Also note that question number 4 B please list each fire only at their highest certification.
Often required depending on exposures
If the insured has Employment Practices Liability (EPL), please complete a new EPL application.
3. Special Events General Survey
If it you has indicated that the department has a special event, needs Liquor Liability coverage or Fireworks Coverage please complete the applicable forms available on our website under the heading Special Events Surveys. Please note, with Special Events, start with the General and Safety and Security Surveys and follow with any additional surveys as indicated by the General Survey.
4. If there are any changes to property values, auto values or any other coverage changes, please submit them with the renewal information.
5. Finally, Continental Western is also requiring an estimator be done on each Fire Hall. Please email us @ erin@ballinsurance.biz or Lois@ballinsurance.biz
if the Fire Hall is covered on the FDK policy and a estimator has not been provided for you.
