Public Entity Pak Renewals


Continental Western requires annually several items for the renewal of their public entity policies. The surveys required for each policy vary depending on the insured’s exposures.

Requied Every Three Years:
1. Public Entity Pak Supplemental Application along with any additional surveys as indicated by the survey for many questions answered in the affirmative.

2. Contact name and phone number

Often required depending on exposures

3. If the insured has auto coverage please provide a drivers list including date of births and drivers license numbers.
4. If the insured has Employment Practices Liability (EPL), please complete and sign a new EPL application.
5. If the insured has any Special Events please complete the applicable forms available on our website. Please note, with Special Events, please begin with the General Survey and Safety & Security Survey and complete any additional surveys as indicated by the General Survey.

6. If the insured has an Umbrella policy, please provide a new completed and signed ACORD application.
7. If the insured has blanket coverage on their property, please provide a new Statement of Values with the insured’s signature.
8. If the insured has made any changes in what they use for the spraying of herbicide and or pesticides from the current policy OR has recently began using pesticides, please complete the herbicide/pesticide survey and submit MSD sheets for each chemical used.
9. Finally, if there are any changes to property values, auto values or any other coverage changes, please submit them with the renewal information.

Please Note! The Worker’s Comp will be audited and the General Liability may be. IMPORTANT REMINDER *** If an employee has more than one job classification the insured needs to keep track of the time or salary amount for each job classification. Just declaring the percent of time will not fulfill requirements of the Worker’s Comp law and the auditor will rate the highest category.
